I come from a family of cross-stitchers, so whenever we wanted to turn a picture into a pattern we’d just find the place that did it as close to what we wanted it to look like. The family favorite is PCStitch, however, I’m partial to Magic Needle (iOS only I think).

Quite a few of them actually. Pixel-stitch.net is the first that comes to mind. I think there’s one called pic2pat. This was done around 1995, I am not entirely sure but it was definitely hand-stitched whether it was stitched by numbers or something similar but definitely not CNC printed.

It reminds me of these things my university teacher used to Funny Peace Love Ohana Stitch Shirt make. I implemented and sold one of these programs around 1998. By the time it was the best tool out there. OPs granny might actually have used it to produce this, but 1995 is a bit too early.
